  • If you do not fulfill or meet the candidacy criteria prescribed for a certification, and wish to apply for the same certification based on your knowledge and work experience, you can fill out this Candidacy Consideration Request Form and attach the required documents (if any) along with this form and submit here.

  • To report any malpractice related to DASCA exams, or any instance of a DASCA credential holder violating the DASCA code of ethics, or to lodge any specific grievance or complaint, you can fill out this Malpractice or Ethics Violation Report Form and submit here.

  • If your application has been picked up for a candidacy audit, you will be required to fill out the Candidacy Audit Response Form and attach the required documents along with this form and submit here.
